Time to Get Your Shot for Upcoming 2018 Fall Flu Season

Woman getting a flu shotIt’s September, so that means flu season will be here soon and it’s time to get a flu shot to keep yourself, your co-workers, patients, loved ones and random folks you come into contact with every day healthy.

That said, here is information on how and where you can get a flu shot on campus at the University of Arizona:

University of Arizona & State Employees

ELIGIBILITY CRITERIA:

  • All Benefit Options Members (enrolled in an ADOA medical plan) age 4 and older including: employees, retirees, spouse, and dependents.
  • State Employees Eligible for Full Benefits (those who were offered ADOA medical benefits, even if they declined coverage)
  • FOR THOSE NOT BENEFITS ELIGIBLE — anyone else, even from off campus – walk-ins welcome — THE CHARGE IS $40.00

TO RECEIVE A FLU SHOT YOU WILL NEED:

  1. Print and fill out the Flu Shot Consent Form
  2. Bring the completed form, your insurance card, and employee ID number

Employee ID Number (EIN) required. This is NOT your Cat Card number. Your EIN can be found on your time sheet.

WEDNESDAY, OCTOBER 10
9 a.m. – 12 p.m. | Keating BIO5, Room 103 (Bdg 240)
THURSDAY, OCTOBER 11 
1:30 – 3:30 p.m. | UA Cancer Center North, 3838 N. Campbell, North campus, Room 1290/1291

All team members, providers, and volunteers must get a flu shot or have an approved medical or religious exemption on file by 11:59 p.m. (EST), Dec. 3, 2018, to continue working for Banner Health.

Why a mandatory flu shot?
